Payment
All customers must fill out applicable information forms irrespective of form of payment. Please pay from invoice. No statement will be mailed. U.S. Orthotics accepts company checks, bank wires, Visa, MasterCard, AMEX and Discover credit cards as payment.

Credit Card
On credit card orders, when merchandise ships, the credit card will be billed. Credit Card authorization form must be signed.

Credit card terms are Point of Sale. Credit card payments made after point of sale are subject to a 3% convenience surcharge.

Open Credit Terms

Net 30 days to credit established accounts, all others are bank wire or credit card. To establish credit please complete our credit application form. Trade references and bank information are required.
